1. Administrative Assistant Position

Job Summary:
The Center for Human Development is a community-based organization that offers services for at-risk youth, individuals, families, and communities in the Bay Area. We are seeking an Administrative Assistant to provide office and reception duties, and coordinate communications and paperwork. The position is 20 to 30 hours per week and is located in Concord.
3 years of related administrative experience and aptitude for accuracy, efficiency, and organization; must have Word processing, spreadsheets, database proficiencies and experience with coordinating communications and logistics.

2. Project SUCCESS Program Coordinator – East Contra Costa County

Project SUCCESS is an evidence-based Alcohol and Other Drug (AOD) prevention program for youth. The Project SUCCESS Program Coordinator facilitates resiliency-focused AOD Prevention Education groups; interactive, activity-based Process groups; and individual Prevention Assessment and Brief Intervention. The Program Coordinator also facilitates school-wide and parent-specific AOD awareness activities and events.
Job Description:
The Project SUCCESS Program Coordinator assists the Program Manager in the implementation of specific programs. This is a full-time (40 hours/week) position, which is based at two middle schools in East Contra Costa County.
A qualified candidate will demonstrate the following characteristics related to successful performance in this position:
  • Associate degree in health, human services, or related field. Bachelor’s degree preferred.
  • Two to three (2-3) years experience with youth and families and experience in substance abuse prevention or related field;
  • Experience working with diverse communities;
  • Demonstrated skills in coordinating and facilitating groups with youth and parents from high-risk environments;
  • Demonstrated skills in working with youth individually, providing crisis intervention and community referrals;
  • Strong written and verbal communication skills, including basic computer skills;
  • Occasional evening and weekend work required;
  • Successful background check and clean TB test;
  • Valid California Driver’s license, reliable transportation, and evidence of insurance coverage.
Starting salary: $16.32 per hour
Benefits include medical, dental, 10 holidays and 19 vacation days per year and a retirement plan (No Employer Contributions).  This is a full-time (40 hours per week), non-exempt position.

3. Health Promoter/Navigator

Job Type: Full-time
In partnership with physicians and other medical staff, co-facilitates patient group visits (diabetes, prenatal, pediatric obesity, and others) at various health center locations
Assists community residents and patients in accessing county and community health services through help in making medical, dental, or mental health appointments at various health center sites.
Educate the community and provide hands on enrollment assistance on MediCal, Covered California, and other county and community health benefit programs through a computer based e-source linkage tool called REACH at various health center sites.
Provides health navigation assistance to Contra Costa Regional Medical Center cancer patients, if assigned. Duties may include: providing reassurance to infusion patients, health navigation, linking patients to resources, assisting patients with forms and applications, working with the Oncology FNP and social worker
Associate degree in community organizing, health, human services, or related field. Excellent position for an individual pursing a degree in health education, public health, community organizing, or other related fields
Knowledge of community health issues
Two to three years of experience working with targeted population
Familiarity with Contra Costa County
