1.  Title: Four Corner’s Program Coordinator

 # of Positions: 1               
Job Type: Full Time 
Salary:  $17.14 per hour
Job Description
The Four Corners Program addresses youth violence, gang involvement, anger management and related family dynamics.  The program provides prevention services that help young people connect to caring adults through positive activities.  Staff provides relevant information regarding the risks and consequences of gang involvement, as well as alcohol, tobacco, and other drug use. The Coordinator works under the Program Director in the implementation of a gang prevention program in Pittsburg. Four Corner program is funded by Pittsburg Unified School District.

Principal Duties:

  • Interview youth who have been referred to the program;
  • Collaborate with school administrators, teachers, and staff on an ongoing basis;
  • Facilitate anger management groups and support groups for both male and female and co-ed participation;
  • Mediate and prevent on-campus violence mediation, on an “as needed” basis;
  • Provide resource and referral information to youth and families;
  • Maintain all data required for reporting purposes, parent permission forms, including program evaluations;
  • Attend staff meetings, trainings, and CHD clinical supervision sessions, as required;
  • Other duties, as assigned by Program Director.

 

Requirements:

A qualified candidate will demonstrate the following characteristics related to successful performance in this position:
  • Associate degree in health, human services, or related field. Work or volunteer experience can be substituted for the required education.
  • Two to three (2-3) years experience with youth and families and experience in gang violence prevention and substance abuse prevention or related field;
  • Experience working with diverse communities;
  • Experience/ flexibility regarding crisis intervention;
  • Demonstrated skills in coordinating and facilitating groups with youth from high-risk environments;
  • Strong written and verbal communication skills, including basic computer skills;
  • Good time management;
  • Self-motivated, team player, good meeting deadlines( i.e. monthly/bye weekly reports);
  • Successful background check and clear TB test;
  • Valid California Driver’s license, reliable transportation, and evidence of insurance coverage.

2.  Title: African American Health Navigator

# of Positions: 1               
Job Type: Full Time (40 hr/wk.)
Job Description
The African American Health Conductors will be part of a team serving African Americans in Contra Costa County. The Health Conductors create a bridge for African American families to the community and public systems that have traditionally been difficult to access due to their complexity, cultural capacity, and trust level. Positions will be based initially in Martinez. Upon completion of the training period, the Health Conductor will be assigned West County Health Center (San Pablo)
Requirements
A qualified candidate will demonstrate the following characteristics for successful performance in this position:
  • Associate degree health education or health studies, health, human services, or related field. Work or volunteer experience may be considered in lieu of degree.  Excellent position for an individual pursing a degree in health education, public health, community organizing, or other related fields.
  • Knowledge of community health promotion and prevention principles, with an emphasis on health for African Americans;
  • Knowledge of MediCal application pro
  • Strong active listening, verbal, and written communication skills;
  • Computer proficiency sufficient to schedule patient visits, input data, create reports and flyers, and receive and respond to emails;
  • High School graduate or GED;
  • Clear TB test;
  • Must possess a valid California Driver’s License and proof of insurance or have access to another form of reliable personal transportation;
  • Ability to pass a Live Scan background check.

3.  Title: Project SUCCESS Clinical Supervisor

 # of Positions: 1               
Job Type: Part Time (1.5 hr/wk.)
Salary:  $75 per session
Job Description
The Clinical Supervisor provides supervision to Program Coordinators of specific programs that provide services to youth. Clinical Supervision is held for 1.5 hours on Thursdays at our Concord office. Supervision is being provided due to the nature of the work, not staff interns accruing hours.
Requirements:
  • Possession of a Master’s degree from an accredited school in social work, marriage and family therapy, psychology, counseling, or related field.;
  • At least two years’ experience providing group supervision;
  • Up to date on Supervisory Training;
  • Experience working with diverse communities;
  • Strong knowledge of group processes
  • Experience working with youth preferred;
  • Ability to hold firm boundaries and hold staff accountable to group agreements;
  • Valid California Driver’s license and evidence of insurance coverage.
